Physical and Health Assessment Protocol – Must be dated no later than 24 months before the last day of your camper at camp. The physical examination or health certificate must include a statement of the child`s general health and the presence of a known medical or emotional illness or disorder that poses a risk to other children or impairs the child`s functional ability to participate in camp safety. Doctor`s Administration Form – THIS ONLY APPLIES IF YOUR CAMPER IS TAKING MEDICATIONS, PRESCRIPTION OR OVER-THE-COUNTER MEDICATIONS. A medication form is required for each over-the-counter and prescription medication brought to camp. Each form must be signed by your child`s health care provider. All medications must be in the original container. The formulary and medications will be picked up at check-in and returned on departure. In order to participate in the camp, all campers must provide proof of insurance. HEALTH PLAN – THIS ONLY APPLIES IF YOUR RV HAS A DISABILITY OR SPECIAL HEALTH NEEDS SUCH AS ALLERGIES WITH PPE PENS, SEIZURES, ETC. If your RV has a disability or special health needs, individual plan care should be developed with the child`s parents and health care provider.
